Clinical Translation

The obstacle is not the physics. It is that nobody writes down the position the knee was in.

What It Shows

The effect's mechanism is settled and its clinical use is not, so the gap between them is where this page sits. 5.19.1 covers the physics of magic angle imaging.

The missing step is small and administrative: the angle between a structure and the field is determined by how the leg lay, and no part of the examination records it. 5.5.1 covers the standard protocol.

It matters because an unrecorded variable cannot be compared, so two studies of the same knee are not comparable on anything the effect touches. 5.20.1 covers structured reporting.

How It Is Acquired

Translating the effect into practice would need three changes, none of which is a scientific one.

The protocol would record the knee's position, as a stated part of the acquisition rather than as a note. 5.5.2 covers the sports knee protocol.

The examination would acquire the structure of interest at more than one position, which is what turns an appearance into a measurement. 5.18.6 covers dynamic MRI.

The report would carry the position alongside the finding, so a later reader can tell what the geometry contributed. 5.20.1 covers structured reporting.

Serial comparison would then be possible, and it is not possible now. 5.18.1 covers quantitative MRI.

Key Learning Points

  • The mechanism is settled and the clinical use is not.
  • The angle is determined by how the leg lay.
  • No part of the examination records it.
  • An unrecorded variable cannot be compared between studies.
  • The three changes needed are administrative rather than scientific.

MRCS Level

How It Is Acquired

In a viva the expected answer is that the barrier to using this effect is methodological rather than physical.

The expected points are the uncontrolled position, the absent documentation, the multiple position acquisition, and the comparability that follows.

Say that the protocol change costs scanner time and no new equipment. 5.5.1 covers the standard protocol.

Say that weight-bearing and dynamic systems already change position deliberately. 5.18.5 covers weight-bearing MRI.

Say that the reporting change is a field in a template. 5.20.1 covers structured reporting.

How Good It Is

Judged as a translation problem this one is unusually tractable, and it has not been translated, which makes the page about incentives rather than feasibility.

Its best feature is that every component exists. Positioning devices, multiple position acquisition and structured templates are all in use for other purposes. 5.18.6 covers dynamic MRI.

It is limited by scanner time, which is the currency every protocol change is paid in and the reason most are refused. 5.1.5 covers the cost-effectiveness of imaging.

It is limited again because the clinical question the measurement would answer, tissue quality, is not one the current pathway asks in a form imaging is expected to answer. 5.1.4 covers imaging pathways.

No figures appear for accuracy on this page, under decision 028, and no angle, scan duration, cost or position count appears either: those are published or local quantities and this environment cannot check them.

Key Learning Points

  • The translation problem is unusually tractable and untranslated.
  • Every component already exists for other purposes.
  • Scanner time is the currency protocol changes are paid in.
  • Tissue quality is not a question the pathway asks of imaging.
  • The page is about incentives rather than feasibility.

FRCS Level

How Good It Is

At decision level the reachable gain is not a new measurement. It is the end of a recurring false positive, and that gain needs no translation at all.

A department that adds the confirming sequence to its tendon and ligament reporting habit removes most of the error immediately. 5.2.14 covers T2 imaging.

A department that records knee position gains comparability for its quantitative work, which is the part most often reported without it. 5.18.2 covers T2 mapping.

Neither change requires evidence that does not exist, which separates them from the measurement question. 5.1.7 covers evidence-based imaging.

The measurement question stays open and this chapter does not pretend otherwise. 5.19.7 covers the current research.

Where It Misleads

The commonest error at this level is treating the whole subject as research when half of it is a reporting habit. 5.20.8 covers common reporting errors.

A quantitative service is offered without position being controlled. 5.18.1 covers quantitative MRI.

Serial maps are compared across visits as though position were fixed. 5.18.5 covers weight-bearing MRI.

A protocol change is proposed without the scanner time being costed, and is refused for that reason rather than on its merits. 5.1.5 covers the cost-effectiveness of imaging.

The reporting template is changed without the readers being taught why the field is there, so it is left blank. 5.20.1 covers structured reporting.

Key Learning Points

  • Half the subject is a reporting habit rather than research.
  • A quantitative service is offered without position being controlled.
  • Serial maps are compared as though position were fixed.
  • A protocol change refused on cost is not refused on merit.
  • A template field nobody understands is left blank.

Fellowship Level

Where It Misleads

At fellowship level this page is the chapter's practical conclusion, and the conclusion is modest on purpose.

The error side of the subject is actionable today, by one habit and one sequence, and that is the whole of what this chapter can offer a patient now. 5.2.20 covers the magic angle effect.

The measurement side needs a protocol nobody runs, for a question the pathway does not currently ask, which is a long way from a clinical service. 5.19.6 covers the cartilage applications.

Anyone proposing the measurement should expect to justify scanner time against an outcome, and the chapter has no outcome evidence to offer them. 5.1.7 covers evidence-based imaging.

The transferable point reaches beyond this effect: a quantitative imaging service that does not record its own acquisition geometry is producing numbers that cannot be compared, whatever the sequence. 5.18.1 covers quantitative MRI.

Key Learning Points

  • The error side is actionable today with one habit and one sequence.
  • The measurement side needs a protocol nobody runs.
  • The pathway does not currently ask the question it would answer.
  • Scanner time has to be justified against an outcome.
  • Numbers without recorded geometry cannot be compared.

References

No reference list is attached to this draft. Evidence verification has not run, because source retrieval was unavailable at generation time. Under the Operations Handbook a missing reference is acceptable and an invented one is a critical failure, so nothing has been cited and no citation marker appears in the text. This page carries no diagnostic accuracy figure, which is decision 028 rather than an omission: the comparative standing of the modalities is standard teaching and a specific sensitivity written from memory is the same class of claim as an invented citation, so ACC-001 refuses the number and the page states the comparison. A reviewer with the literature can add the figures into How Good It Is without restructuring the page. Any measurement here, including a sequence parameter, an angle or a millimetre, is written from standard teaching, has not been checked, and is registered under decision 006 in pipeline/config/figures with a verification state of UNVERIFIED_FROM_MEMORY.
