What It Shows
A bursa is a fluid filled space that reduces friction between moving structures, and the knee has several. 5.5.11 covers the synovium.
Some communicate with the joint and some do not, which determines whether a joint effusion distends them. 5.4.4 covers the posterior knee on ultrasound.
It matters because a small amount of fluid in most of these spaces is normal, and reporting it as a finding is one of the commonest sources of noise in a knee report. 5.1.6 covers imaging pitfalls.
How It Is Acquired
Assess on the fluid sensitive sequences, where fluid is conspicuous. 5.2.16 covers fat suppression.
Know which spaces normally contain a little fluid and which do not. 5.5.11 covers the synovium.
Assess the suprapatellar region as a continuation of the joint rather than as a separate bursa, since it communicates. 5.5.1 covers the standard protocol.
Assess the semimembranosus and medial gastrocnemius interval, where a popliteal cyst arises. 5.4.4 covers the posterior knee on ultrasound.
Distinguish simple fluid from fluid with internal content, which changes the interpretation. 5.14.7 covers septic arthritis.
What Normal Looks Like
A small amount of fluid in the suprapatellar region, continuous with the joint. 5.5.1 covers the standard protocol.
A trace of fluid in the pes anserine and other common bursae.
No fluid in the prepatellar space in an asymptomatic knee.
Clean fluid without septation or internal echo equivalent. 5.14.7 covers septic arthritis.
Key Learning Points
- A bursa reduces friction between moving structures.
- Some communicate with the joint and some do not.
- A small amount of fluid in most of these spaces is normal.
- Reporting normal fluid is a common source of noise.
- A popliteal cyst arises between two named tendons.
MRCS Level
How It Is Acquired
In a viva this is the tissue to name when asked what is over-reported on knee studies.
The expected points are the fluid sensitive sequences, which spaces normally contain fluid, the suprapatellar continuity, the popliteal interval, and the internal content.
Say that a small amount of bursal fluid is usually normal. 5.1.6 covers imaging pitfalls.
Say that a popliteal cyst implies an intra-articular cause. 5.4.4 covers the posterior knee on ultrasound.
Say that the clinical picture decides whether a distended bursa matters. 5.1.2 covers clinical examination versus imaging.
What Normal Looks Like
The reasoning expected is that a bursa exists because two structures move against each other, so a small amount of fluid in it is the normal consequence of its function. 5.5.11 covers the synovium.
A communicating bursa fills when the joint has an effusion, so its distension is a sign of the joint's state rather than a separate problem. 5.5.1 covers the standard protocol.
A popliteal cyst is joint fluid pushed through a valve like neck, which is why it indicates an intra-articular process and recurs if that is untreated. 5.4.4 covers the posterior knee on ultrasound.
A non-communicating bursa distends from its own irritation, which makes it a local diagnosis rather than a joint one. 5.4.1 covers the anterior knee on ultrasound.
How Good It Is
Magnetic resonance imaging detects bursal fluid with high sensitivity and establishes its significance not at all.
Its best contribution is identifying a distended non-communicating bursa as a local process, and a popliteal cyst with its neck. 5.4.4 covers the posterior knee on ultrasound.
It is limited because the finding is common in asymptomatic knees, so the clinical picture carries the interpretation. 5.1.6 covers imaging pitfalls.
No figures appear for accuracy on this page, under decision 028, and no fluid volume or bursal size thresholds appear either: those are published quantities and this environment cannot check them.
Key Learning Points
- A small amount of fluid is the normal consequence of a bursa's function.
- A communicating bursa's distension is a sign of the joint's state.
- A popliteal cyst is joint fluid pushed through a valve like neck.
- A non-communicating bursa distends from local irritation.
- The finding is common in asymptomatic knees.
FRCS Level
How Good It Is
At decision level a bursal finding matters when it matches the symptoms and not otherwise.
A distended non-communicating bursa with local tenderness is a local diagnosis with a local answer. 5.4.16 covers the knee injection on ultrasound.
A popliteal cyst indicates an intra-articular cause, and treating the cyst alone invites recurrence. 5.4.17 covers aspiration on ultrasound.
A complex collection with internal content raises infection and changes the urgency. 5.14.7 covers septic arthritis.
Incidental bursal fluid in an asymptomatic area needs no action and ideally no mention. 5.20.1 covers structured reporting.
Where It Misleads
The commonest error is reporting normal bursal fluid as a finding. 5.1.6 covers imaging pitfalls.
A communicating bursa's distension is reported as a separate diagnosis. 5.5.1 covers the standard protocol.
A popliteal cyst is reported without its intra-articular cause being sought. 5.6.3 covers horizontal meniscal tears.
A complex collection is assumed to be simple fluid. 5.14.7 covers septic arthritis.
The symptoms are attributed to a bursa that does not match them. 5.1.2 covers clinical examination versus imaging.
Key Learning Points
- Normal bursal fluid is reported as a finding.
- A communicating bursa's distension is reported as a separate diagnosis.
- A popliteal cyst is reported without its cause.
- A complex collection is assumed to be simple.
- Symptoms are attributed to a bursa that does not match them.
Fellowship Level
Where It Misleads
At fellowship level the bursae are the clearest example of a sensitive modality generating findings that add nothing.
A fluid sensitive sequence detects small volumes of fluid everywhere they occur, and they occur normally, so the modality's sensitivity produces a list rather than a diagnosis. 5.2.16 covers fat suppression.
Each such item in a report has to be read, considered and often explained to a patient, which is a cost the report imposes without a corresponding benefit. 5.1.6 covers imaging pitfalls.
The exception that justifies the attention is the popliteal cyst, whose neck locates it and whose cause is the actual finding, and reports frequently give the cyst without the cause. 5.4.4 covers the posterior knee on ultrasound.
Ultrasound characterises these collections faster and can aspirate them in the same sitting, which makes it the better modality for a bursal question asked on its own. 5.4.17 covers aspiration on ultrasound.
The disciplined practice is to report what matches the symptoms and to say explicitly when fluid is within normal limits. 5.20.1 covers structured reporting.
Key Learning Points
- Sensitivity produces a list rather than a diagnosis.
- Each reported item imposes a cost without a benefit.
- The popliteal cyst's cause is the actual finding and is often omitted.
- Ultrasound is the better modality for an isolated bursal question.
- Say explicitly when fluid is within normal limits.

References
No reference list is attached to this draft. Evidence verification has not run, because source retrieval was unavailable at generation time. Under the Operations Handbook a missing reference is acceptable and an invented one is a critical failure, so nothing has been cited and no citation marker appears in the text. This page carries no diagnostic accuracy figure, which is decision 028 rather than an omission: the comparative standing of the modalities is standard teaching and a specific sensitivity written from memory is the same class of claim as an invented citation, so ACC-001 refuses the number and the page states the comparison. A reviewer with the literature can add the figures into How Good It Is without restructuring the page. Any measurement here, including a sequence parameter, an angle or a millimetre, is written from standard teaching, has not been checked, and is registered under decision 006 in pipeline/config/figures with a verification state of UNVERIFIED_FROM_MEMORY.
